whakamārama mō tēnei tūranga | About the role

As an Advanced Support Worker at Access Community Health, you will be part of an exciting shift in how our support is delivered across the Central Region. This newly created role gives you the opportunity to work closely with our team, tāngata and whānau to support the delivery of flexible funding workstreams and person-directed services.


In this role, you will champion Enabling Good Lives principles in everyday service delivery and support the growth of Individualised Funding across the region. You will liaise with tāngata, whānau, colleagues and partner agencies to ensure support is well communicated, coordinated and centered on what matters most to the person.


You will also work alongside tangata with more complex support needs and carry out tasks, for which you will have received training and competency verification. You will also provide leadership, on-the-job guidance and just-in-time support to less experienced staff, helping to develop and strengthen our support workers in practising whakamana tāngata and enabling good lives.


Ngā tohungatanga mō tēnei tūranga | About you

  • Good understanding of disability and Enabling Good Lives principles
  • Genuinely and reliably committed to working with others to achieve great support outcomes
  • At least two years’ experience delivering disability, home-based or community support services.
  • Positive, can-do attitude and excellent interpersonal skills
  • Able to work effectively within multi-disciplinary teams (e.g., Palliative Care, Home-Based Nursing, Support Worker Coordination).
  • Level 4 Health and Wellbeing Certificate (or equivalent experience)
  • Proficient in using Microsoft Office suite.
  • Must have a full New Zealand driver's license.
