ABOUT THE ROLE

We are looking for an organised, confident and people-focused Care Coordinator to join our Referrals and Triage team based in the heart of Mt Eden Village. In this pivotal role, you will help manage the front door to our ICPMSK programme—receiving and screening referrals, connecting with patients, gathering the information needed for informed triage, and coordinating timely next steps with internal team and across our provider network.

Your work will help create a clear pathway from the first referral, improve timeliness and continuity, reduce duplication and rework, and give patients a better understanding of what happens next. Success in the role means well-managed referrals, reliable records, early identification of barriers and consistent communication with key providers, ACC and internal teams.


WHAT YOU WILL BE DOING

  • Receive and review referrals and supporting information for accuracy.
  • Check claim status, ICPMSK eligibility and potential pathway requirements.
  • Contact patients to explain the service, confirm relevant details, consent and support early engagement.
  • Gather accurate clinical, functional, work and contextual information to support triage and pathway decisions.
  • Coordinate pathway entry and onward navigation to appropriate Careway services.
  • Prepare clear, accurate documentation and correspondence within agreed timeframes.
  • Work collaboratively with the interdisciplinary team and escalate clinical or pathway concerns promptly.
  • Contribute to service quality, referral-flow and continuous improvement.
  • Maintain patient privacy, cultural safety and professional communication at every touchpoint.


ABOUT YOU

You are organised and curious individual with eye for detail confident communicating with a range of people who follows through, asks good questions and takes pride in helping clients and stakeholders get clarity. You have experience in similar roles across healthcare, recovery and rehabilitation, insurance, customer service, administration or reception role, retail or another people-focused environment.


KEY SKILLS AND EXPERIENCE

  • Experience in healthcare coordination, referrals, rehabilitation, ACC services, medical administration or a comparable patient-facing role.
  • Clear, confident communication by phone, email and written documentation.
  • Confidence building productive relationships with clients, employers, providers and ACC.
  • Attention to detail, sound judgement and confidence working with clinical and administrative information.
  • Practical problem-solving skills and sound judgement in complex or time-sensitive situations.
  • Ability to work independently and autonomously while knowing when to escalate or seek support when information or next steps are unclear.
  • Competence using Microsoft 365 tools, including Teams, Outlook, Excel and Word.
  • A commitment to culturally safe, equitable and person-centred service delivery.
  • Clinical knowledge of ACC, musculoskeletal rehabilitation or interdisciplinary care would be an advantage, but it is not essential.
